Consistent, high quality is the constant goal of high reliability organizations. Here's a quote by W. Edwards Deming, considered by many to be the father of modern quality improvement science, regarding accountability for quality outcomes within an organization: "Quality is everyone's responsibility." This means everyone is responsible - at least for their own conduct - in contributing to high quality care.

"The greatest waste is the failure to use the abilities of people, to learn about their frustrations, and about the contributions that they are eager to make."
One of the major goals of using a fundamentally-sound and consistent approach to quality improvement work is to ensure that everyone involved in the activity has the opportunity to use their abilities for the common good... to listen to others, and to be heard and supported. Taking this approach engenders confidence in the group's activities and improves the likelihood of a meaningful implementation of the group's plans. So, please do listen to one another.
